Office Policies

Copays and deductibles are payable at the time of service. Cash, credit card, or personal checks are accepted. Most of our client services are covered by your health insurance plan. The Velis Vein Institute participates in some health insurance plans and not in others. In plans which we do not participate, services are usually covered as an “out-of-network” provider. This means different rules and different rates of reimbursement, depending on the plan. Once again, not all services are covered. Please check with your health plan for your benefits. As a service to our valued clients, we will bill the insurance company for you. In addition, we will facilitate the process by providing you with an itemized bill, including insurance codes (CPT and ICD-9 codes).
